Referral Process

The SABP will receive referrals from a variety of sources, but normally they will come from:

• Social worker
• Teacher
• Health Visitor
• Psychologist
• Any other professional involved in the young person’s life

Referring Agency puts forward request for referral Form

Individual Caseworker completes referral form

Form sent to project co-ordinator

Referral Accepted or Screened Out

Home visit by Project Co-ordinator & referring Agent

Young person prioritised & placed on waiting list- contact maintained with referring agent
