The throughcare support team
work with young people aged between 15-21 years who
are/ were Looked After by the local authority and young
homeless. The Team currently supports a caseload of
over 70 young people and offers a range of services
including:
Individual contact and crisis support
Financial support
Groupwork/drop-in facilities
Practical and emotional support
Information and links to advocacy
services
Evening and week-end support
Mediation services
Support at an early stage of presentation.
Continuous assessment process involving
regular reviews of young people’scircumstances.
Family Mediation
All young people Looked After
away from home including young people in foster care
placements should be referred to the Throughcare Team
at the age of 15 years. This then allows throughcare
in conjunction with field work social workers, key workers
and the young person the opportunity to plan and prepare
for the time when the young person is no longer Looked
After. Section 29 of the Children (Scotland) Act 1995
states that all young people leaving care should have
a comprehensive needs assessment carried out which then
informs a detailed Throughcare/ Aftercare plan.
Throughcare planning should focus
on the following areas:
Accommodation and housing support
Self esteem , Social Skills and
leisure
Employment/ Training. Hopes and
aspirations
Health needs and family support
Alcohol, drug use and offending
Throughcare Support Staff are actively
involved in other areas of work including:
Youth Housing Support Group
Screening and Resource Group
South Carrick Youth Issues Group
Scottish Throughcare and After
Care Forum and the Landsbrough Project
